Registration forms are available in PDF, Word and SXW formats. Registrants are asked to complete the form and to send it by postal mail, along with a check for the correct amount, payable to "HSEMB," to the society's mail address:

Registrants are encouraged to take advantage of the early registration discounts. The completed registration form and payment must be received by 17 December 2004 for the Society to honor this discount.
